Instruction to your Bank or Building Society
Please pay BPS re: SOUTHAMPTON HOSPITALS Direct Debits from the account detailed in this Instruction subject to the safeguards assured by the Direct Debit Guarantee. I understand that this Instruction may remain with BPS re: SOUTHAMPTON HOSPITALS and if so, details will be passed electronically to my Bank/Building Society.

NOTE: Banks and Building Societies may not accept Direct Debit Instructions for some types of accounts.

The Direct Debit Guarantee

  • This Guarantee is offered by all banks and building societies that accept instructions to pay Direct Debits
  • If there are any changes to the amount, date or frequency of your Direct Debit BPS re: SOUTHAMPTON HOSPITALS will notify you 10 working days in advance of your account being debited or as otherwise agreed. If you request BPS re: SOUTHAMPTON HOSPITALS to collect a payment, confirmation of the amount and date will be given to you at the time of the request
  • If an error is made in the payment of your Direct Debit, by BPS re: SOUTHAMPTON HOSPITALS or your bank or building society, you are entitled to a full and immediate refund of the amount paid from your bank or building society. If you receive a refund you are not entitled to, you must pay it back when BPS re: SOUTHAMPTON HOSPITALS asks you to.
  • You can cancel a Direct Debit at any time by simply contacting your bank or building society. Written confirmation may be required. Please also notify us.

If you are a UK tax payer we could claim Gift Aid on your donation and receive an extra 25p for every £1 you give!

YES I am a UK tax payer and I wish Southampton Hospitals Charity to claim Gift Aid on all donations I have made over the past four years and all future donations.

I understand that if I pay less income tax and/ or capital gains tax in the appropriate tax year than the amount of Gift Aid claimed on all my donations, it is my responsibility to pay any difference.

You can cancel your declaration at any time, and you must inform us if your name, address or tax status changes.

If you pay Income Tax at the higher or additional rate and want to receive the additional tax relief due to you, you must include all your Gift Aid donations on your Self-Assessment tax return or ask HM Revenue and Customs to adjust your tax code.
